Stages of change

This worksheet introduces the Stages of Change model, a framework designed to help individuals understand their readiness for change.

It guides parents/carers through the five stages (using accessible language) to help them identify where they currently stand in relation to a specific goal or habit they wish to change. By recognising which stage they are in, parents/carers can set realistic expectations and take appropriate steps towards meaningful progress.
