5Ps Formulation Template for Young People
This young-person-friendly 5Ps formulation template provides a structured framework for collaborative CBT case formulation. It helps therapists and young people explore how difficulties developed, what may be maintaining them, and which strengths and protective factors can support change. The template can inform assessment, shared understanding and treatment planning.
