This blank journey-mapping template is designed to follow on from the example worksheet and can be used as a structured homework activity. It supports young people in reflecting on significant life events, placing them on a timeline from early experiences to the present.

The resource is designed to help therapists and young people collaboratively build a formulation by identifying key influences, triggers, and turning points, while encouraging self-reflection and discussion in subsequent sessions.
