Keeping well plan

This two page worksheet is designed to support young people as they transition out of therapy, providing a structured plan for maintaining their progress independently. It it designed to serve as a reminder of the skills they have learnt and encourages them to continue using these techniques when challenges arise.

By recognising when things become difficult and knowing what steps to take, this resource is designed to help therapists empower young people to maintain their well-being and build resilience beyond therapy.
