Stages of change
This worksheet introduces the Stages of Change model, a framework designed to help individuals understand their readiness for change. It guides young people through the five stages (using accessible language) to help them identify where they currently stand in relation to a specific goal or habit they wish to change.
By recognising which stage they are in, young people can set realistic expectations and take appropriate steps towards meaningful progress.
