Activity and mood log (2 pages)

This self-monitoring tool is designed to help young people record their daily activities and track fluctuations in their mood throughout the day. It provides a structured layout for recording mood ratings in the morning, afternoon, and evening.

This two page version provides further space for recording activities.

By consistently logging activities and associated mood ratings, users can gain insight into patterns and triggers, empowering them to better understand their emotional well-being and identify strategies that support positive changes.
